Running VERY Rich after new injectors and ESS Tune??

After several years on my wish list, I finally bought a brand new ESS VT2-525 supercharger and got it installed. I was previously running an Evolve tune with no major issues. Occasionally, my car would "hunt" on idle and once or twice even stalled (this was very random and very seldom). Fast forward, I get the ESS tune uploaded and fire up the car and I and I am running about 12:1 AFR WITH maxed out STFT at -30.5. ESS said the tune is correct and thought it could be my aftermarket Status Gruppe headers so they recommended I put back on my OEM headers. I did, and still maxed STFT and running rich! I am awaiting getting my aftermarket wideband sensor/gauge installed, however, all signs point to be ACTUALLY running rich, and not just showing it (all 4 O2 sensors indicate rich, my plugs are fouling, STFT is maxed at -30.5, and AFR shows around 12). The guys at ESS have been helpful but I feel like they are just kind of guessing around at this point.
I have checked and cleaned the plugs and coils and there are no codes.

Has anyone experienced anything similar, with or without a supercharge? I have considered bad O2 sensors, but the likelihood that all 4 of them failed seems very low. I also wondered about a bad fuel pressure regulator, but I feel like a failed regulator would result in low pressure and running lean.

They are the Bosch grey 012 injectors that come with the ESS supercharger... not sure of exact part#.

Negative. Pensacola, FL. ESS wants me to take it to their dealer in Orlando... 7hr drive and god knows how much to have him tear it all apart to figure it out...

Do you have any MAF error codes? This is one way to be certain the alpha-n tune is loaded in place of the stock tune without pulling the file. How are you data logging?

Negative. Until just recently I had zero codes at all. The MAF was cut out and the wires for it wired into what ever the sensor is with the supercharger. The codes that are now throwing are a a heater O2 for B1S2 and B2S2. ESS wanted me to clear adaptation by disconnecting all 4 sensors and starting the car. After reconnecting all sensors and clearing all codes, the two downstream heater codes came back.

I use the Torque Pro app and a bluetooth OBDII.
