Just had a thought. I think I'll remove the water pump belt and let the car sit overnight. Then in the morning I can turn the water pump pulley by hand, isolated from the rest of the engine, and see if I get the noise.

I usually get to the tensioners from below (certainly the A/C one). Hope I can get to the top tensioner from above.

(belts and 2 of 3 pulleys were done at 50K miles)
