Consider Direct Primary Care (if available in your area) with an attached free generic plan (which should cover about 80% of your needs), and a catastrophic rider. Dental and vision cost pennies. Depending on your census you'd be in for less than 160 per person per month for singles and about 210 to 240 for a family of 4.
