At least on my car, the former (easing into gear) doesn't work well. If I ease or slow movement into 2nd, I'll likely "crunch" 2nd. The consistent smooth movement (not necessarily slow) works best.

I always do it in 2 motions, with just the slightest hesitation in between.
One to pop it into neutral, and one to put it into the next gear.

The only time I get a grind (about once per year) is when I get too nonchalant and let my hand slip.
I don't get it all the way into gear, but my left foot's not interrupt driven, so that subroutine continues to execute, and I let the clutch out with it 1/2 in gear.
