P1140 Code - Maf

Changed the headers on my wife's car a few weeks ago.
Now, a cel is popping up, P1140 - MAF sensor out of range.

What have you guys seen before to cause this? I am guessing a vacuum leak - but not 100% sure. Looking for a few pointers on where to start. No other codes present, and my scanner is the generic reader.

Runs beautifully.

I did pull the intake (not the manifold) for motor mount hardware replacement. But, I have triple checked the area. It has to be the culprit.
I have an ultrasonic tester, and will see if I can find it....

Cant find it...
Unplug the MAF and nothing changes in idle characteristics.

Have you tried resetting it?

Unplugging it will put the car in speed density mode and throw a code... at least, that's what it does on my 3.0i. If the orientation was incorrect, the idle would be erratic. I'd have to look at the M again, but on the 3.0i it is definitely possible for the plug to be reversed (plugged in upside down). Check the wiring thoroughly for loose wires. Otherwise, you could check the M3 forums.

Can't put it in upside down.
I have reset it, but it comes back in about 100 miles.
